Question: How do I correct the error, "Forms Engine Failure" after updating to OS Sierra?

Answer: This is a known issue with OS Sierra. Currently to correct this:

• Open your hard drive and go into your Applications folder

• Go into the Form Viewer folder located in Applications.
  

• Copy the Form Viewer  application icon  to the desktop. **NOTE - Do not make a shortcut on the desktop but make a copy of the program icon

• Once you have the Form Viewer application icon on the desktop drag it back into the Form Viewer folder in Applications. When prompted that it already exists select Replace.

• Preview the report.
